ProCare Hospice is a Medicare Certified and CHAP-Accredited program committed to providing quality of life for those who have a limited life expectancy.

We believe the patients are the center of our hospice and palliative care, and have the right to spend their days pain-free and with dignity.  The foundation of our patient care is achieved by listening and responding to the needs to the patient and family.

Procare Hospice uses a family-centered approach to end of life care for the patient and family. Our goal is to provide care for the patient and their families in an atmosphere in which remaining life can be completed with maximum independence, comfort, compassion and dignity.

We use an interdisciplinary team approach. The team consists of:

  • Patient and family
  • Hospice Medical Director
  • Primary Physician
  • Specialized Hospice Nurses
  • Medical Social Services
  • Spiritual Counselor
  • Certified Hospice Health Aid
  • Volunteers

This team approach encourages the patient, family members, and the professional team to exchange knowledge and communication on the treatment benefits and preferences.  By the way, our Hospice nurses are available 24x7x365.

As a unit of care, the team, including the patient and family, anticipate problems and address them, rather than waiting for issues to manifest. The interdisciplinary team will coordinate and manage the care across all settings including the transition to hospice is necessary. The patient/family as a unit will work closely with the hospice staff. Together, they will actively participate in developing a plan of care that attains the outcomes that reflect the personal wishes, values and goals best suited for the particular needs of the patient.

The Hospice Physician directs the patient care. The patient may still be seen by his or her primary physician but treatments are coordinated through Hospice. Hospice Team Members include:

  • Social Service
  • Spiritual Care
  • Home Health Aide and Volunteers

Additional services may include:

  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy
  • Respiratory Therapy
